Your Privacy Rights Under HIPAA

Mid-Cities Medical takes your privacy seriously and the Health Insurance Portability and Accountability Act (HIPAA) gives you rights to protect your personal health information. We respect your rights and are committed to keeping your health information safe.

1. You can see your health records

  • You have the right to look at your medical records and ask for a copy, paper or digital.
  • We will provide them within 30 days, unless special circumstances prevent it.

2. You can ask us to fix mistakes

  • If something in your record is wrong or missing, you can ask us to update it.
  • We will review your request and determine whether we can make the changes, responding in writing.

3. You can limit what we share

  • You will receive a Privacy Notice that explains how we use and share your information, including your options to limit that sharing.
  • You can ask us not to share certain details for treatment, payment, or healthcare operations.
  • If you have paid in full for a service, you can ask us not to share information about that service with your health insurance.

4. You can choose how we contact you

  • You can choose how we contact you; E.G., by sending mail to a different address or calling/texting a number you prefer.

5. You can ask who we have shared your info with

  • You can ask for a record of when and with whom we have shared your health information, except for routine purposes like treatment or billing.

6. You can speak up if you think your privacy was violated
