- You can choose how we contact you; E.G., by sending mail to a different address or calling/texting a number you prefer.
5. You can ask who we have shared your info with
- You can ask for a record of when and with whom we have shared your health information, except for routine purposes like treatment or billing.
6. You can speak up if you think your privacy was violated
- You can file a complaint with us or with the U.S. Department of Health and Human Services, without fear of retaliation.
