VA News – Many patients leave an appointment only to realize they forgot to mention a symptom or ask about a new medication, but bringing a list can help you stay organized and make the most of your time with your provider.

  • Begin your list as soon as you schedule the appointment. Keep it in one consistent place, like a notebook or phone app, so you can add concerns as they come up.
  • Describe symptoms with details – what it feels like, when it happens, how long it lasts, and what makes it better or worse. Specifics help your doctor assess things more quickly.
  • Put your two or three most important issues at the top to make sure they’re addressed first, even if time is limited.
  • Write down the exact names and doses of all medications, vitamins, or supplements, along with any questions about side effects or interactions.
